It was acute care and Mayo's reputation.

I rented an efficiency apartment for $530/month. They were very flexible with my move out dates. For the most part living there was mostly without incident. (except that my landlord took it upon herself to CLEAN my apartment for last-minute showing she gave me about 4 hours notice for -- and I was out of town... creepy!) I lived a mile away from St. Mary's Hospital and I walked most days.
Describe types of conditions or diagnoses, as well as age ranges exposed to during this fieldwork.
You see everything but your experience is divided between the orthopedic floor, general medicine, and cardiac. Ages for me were mostly 40-80 years, but I think the range went from 23-90-something.
What types of assessments and interventions were used?
MoCA, Short Blessed, SLUMS, CPT, Allen, CAM, EFPT. You'll get proficient in all of these, or at least most of them. (and you'll have opportunities to practice as well)
What are your thoughts on the site's environment for the employees and students? (Please consider: organization, collaboration, mentoring, learning opportunities)
Amazing. Mayo participates in the collaborative learning model, so you will have a CI who's primary job is to be your CI -- and it's fantastic. You also will get to work with 2-3 peers depending on your setting. Also you have your OWN desk, which is super helpful.
Please comment on personality traits and/or preparation that would facilitate success at this site.
Be motivated to work long hours and learn! Don't be competitive with your peers. Take advantage of everything Mayo has to offer. If this is your first fieldwork, you will definitely be spoiled by the facility, accommodations, experience.

Definitely take advantage of Mayo's work out facilities. Rochester is NOT a happening town.
